Referrals

Are you working with a young person or family that you think would benefit from joining Healthy Habits?

Referrals from professionals should be completed through the online referral portal: School Health Service (East Sussex) referral form.

If you’re a healthcare professional, it would be preferable that you weigh and measure the child and calculate their BMI centile using the NHS BMI calculator, you can then include this information on the referral form.

If you’re not sure about a referral, you can contact out School Health One Point for more information or to discuss a potential referral further.

What happens once you’ve made a referral?

  • Referral triaged in our School Health One Point
  • Parent/carer will then be contacted by a School Nurse to understand their needs and the best route for them
  • Parent/carer will then be contacted by a member of the Healthy Habits team to enrol onto a group and complete a pre-assessment.
